Participant Waiver, Release of Liability & Media Consent

By registering for and participating in the Elite Basketball Clinic featuring Metta World Peace at The Hoop Vancouver, I acknowledge and agree to the following:

1. Assumption of Risk & Release of Liability

I understand that basketball and related physical activities involve inherent risks, including potential injuries. I voluntarily accept these risks and agree that The Hoop Vancouver, its owners, staff, coaches, instructors, and event partners shall not be held liable for injuries, accidents, or damages arising from participation in the clinic, to the fullest extent permitted by applicable law, including claims arising from negligence.

2. Photo & Video Consent

I understand that photographs and videos may be taken during the clinic. I grant The Hoop Vancouver permission to capture and use the participant's image, likeness, and voice in photographs, videos, and other media for promotional, advertising, marketing, and social media purposes, without additional compensation or prior approval.

3. Medical & Emergency Consent

I confirm that the participant is physically fit to participate in basketball activities. In the event of an emergency, I authorize The Hoop Vancouver staff to seek appropriate medical assistance if necessary.

4. Parent / Guardian Acknowledgment

For participants under 19 years of age, this waiver must be reviewed and acknowledged by a parent or legal guardian.
